The customer online survey will take just a few minutes to complete. Here are the steps to get started.
- Make a Purchase
Make a purchase at Shoppers Drug Mart and keep the receipt.
- Go Online
Visit either www.surveysdm.com or www.surveysdm.ca.
- Receipt Code
Enter the 15-digit Certificate Number on Shoppers receipt.
- Store Location
Select your store location.
- Date
Indicate the date of your most recent shopping trip.
- Take the Survey
Follow the instructions to complete the survey.
- Register for the Contest
Select if you would like to be entered into the contest and provide your contact information.
Pro Tips:
- To enter by phone, call 1-800-701-9163 and complete the Survey via Interactive Voice
- If you don’t have a receipt but still want to enter the Contest without purchase, follow the instructions to select “Don’t have a Receipt” on the website. Then, proceed to complete and submit the Survey on the Contest entry page. You will receive one entry into the applicable Monthly Draw Period.
What Are The PC Optimum Survey Prizes?
There will be 24 prizes (2 per Monthly Draw Period) available to be won during the Contest Period. Each of the two monthly prize winners will receive their choice of either:

You will need a store receipt issued within the last 7 days and the 15-digit Certificate Number found on your sales receipt.
That’s the 15-digit number that you can find next to “CERTIFICATE NUMBER” at the bottom of your sales receipt.
You can enter online, by phone, or without purchase up to 5 times per Monthly Draw Period.
