How To Complete The Survey
The customer survey will take just a few minutes to complete. Here are the steps to get started.
- Make a purchase at Shoppers Drug Mart and keep the receipt.
- Visit either www.surveysdm.com or www.surveysdm.ca.
- Enter your 15-digit Certificate Number found on your sales receipt. If you don’t have a receipt, click HERE to take the survey.
- Select your store location.
- Indicate the date of your most recent shopping trip.
- Take the survey.
- Select if you would like to be entered into the contest and provide your contact information.

Contest Rules
- Eligibility: Contest is open to legal residents of Canada who are the legal age of majority or older in their province/territory of residence at the time of entry.
- Contest Period: This Contest begins on January 1, 2025 and ends on December 31, 2025.
- Limit: Limit 5 entries per person per Monthly Draw Period.
